Clarendon County Disabilities and Special Needs Board offers Case Management services to assist individuals and families with disabilities to access support and services through program referral, needs assessment, coordination of resources, advocacy, and monitorship of services. Based on the individual's interest, the Case Management program offers the following:
ASSESSMENT - The Case Manager performs activities to obtain and review information in order to determine the strengths, preferences, resources and needs of the eligible person.
CARE PLANNING - The Case Manager, individual, family and friends develop a plan to ensure that all needed services are identified. Together the team will identify a course of action to respond to the assessed needs. The goal is to help all eligible individuals to be as independent as possible.
REFFERAL AND COORDINATION - The Case Manager helps individuals and families find appropriate services. This involves conferring with service providers and other professionals to gain a better understanding of a person's current situation and to determine the best course of action to address identified needs.
MONITORING AND FOLLOW-UP - The Case Manager reviews identified needs on a routine basis to ensure all areas are being addressed. The Case Management staff also, with the help of the eligible person, addresses any new needs that arise and assist with finding a way for the Individual to be successful
FAMILY SUPPORT SERVICES - Individual/family support services often enable people with disabilities to remain in their own homes.
